• Complete the brief information sheet
• Identify the donation designation section and designate other
• Put Business School donation in the Other Section
• Identify one of the following specific scholarship funds to donate to, and put the name of the fund in the Other Section also
• Gary Giles Scholarship Fund
• Jerry Horgesheimer Memorial Scholarship Fund
• School of Business General Scholarship Fund
• School of Business Technology Fund
