Administrative/Supervisory Internship Application

 Procedure for  Application and Completing Administrative Internships

All required administrative courses must be completed prior to beginning the internship. No grade of "C" or below will be accepted .

Only one administrative internship will be granted the first semester, unless you submit proof from your school district that you have taken a leave of absence.  You may take two internships the second semester.  Elementary and Secondary internships are available only in the Fall and Spring semesters. Capstone can be completed during the summer session.  It is required that the criteria for each internship be completed by the end of finals each semester.  No incompletes will be given.   Should you not complete the necessary hours and work during the semester, you will be required to enroll in EDUC 6931 Capstone Seminar for 1 credit until you have completed the necessary requirements.  A pass or fail grade will be issued when all course criteria has been met.

Submit an Administrative/ Supervisory Internship Application below, Mentor Internship Acceptance Form http://suu.edu/ed/masters/AdminMentorAcceptForm.html and 1099 Form http://suu.edu/ed/masters/1099form.html the semester before you propose to begin your internship hours. Return the form either by fax 435.865.8046 or mail to the address below prior to the deadline dates listed below.  Remember no internship hours can be done before you are enrolled in the internship class or before the Office of Graduate Studies has received your Administrative Mentor Acceptance Forms. 

Graduate interns are responsible to contact site administrators in their districts to get their permission to complete an elementary and secondary internship at their site (no mentor is designated for the capstone internship). The mentor acceptance form must be signed by each internship mentor principal and returned to Office of Graduate Studies in Education along with the application and before registration will take place. No more than two SUU graduate interns will be allowed at any one school at any one time. 

After internships are approved and mentor acceptance given, it is the interns responsibility to deliver a course syllabus to the principal and discuss a plan to accomplish the objectives of the internship.  It will be the intern's responsibility to make sure mentors have the midterm http://suu.edu/ed/masters/Mid-termReport.html and final evaluation form at the appropriate time to evaluate them and see that they are returned to SUU. 

All approved applicants will be registered for EDUC internship requested by the Office of Graduate Studies in Education.  Applicants cannot register through the registrar's office at SUU or on line.  It will be the intern's responsibility to see that the cashier's office is paid the amount owed for the internships.

Administrative Intern candidates will be assigned a University Supervisor to track their progress in achieving the objectives of the internship and completion of all required assignments (see course syllabus). All required course criteria must be forwarded to their assigned University Supervisor.

Once the first internship is completed it is the intern's responsibilty to notify the Graduate Office that they need to be registered for the next internship by submitting the Internship Permission Form http://suu.edu/ed/masters/InternshipPermissionForm.html by the deadline dates listed below. 

You will need to take the PRAXIS Educational Leadership Administration & Supervision: Test Code 0410. Schedule the examination during your final internship.  You will need to have the results sent to SUU Graduate Department (A4092 college code) and USOE (R8375 state code) so the score can be posted to your completed file. At this time there is not a Pass/Fail score.

Once you have completed the entire program you will have an exit interview with your University Supervisor.
