Forms that must be submitted to the University Supervisor:

• Current Contact Information
• Verification of Handbook and Conceptual Framework Discussion with Mentor Teacher(s)
• Student Teaching Schedule
• Student Teaching Daily Accountability and Personal Reflection
• Teacher Candidate Master Accountability Form

Teacher Work Sample (TWS) must be submitted to the University Supervisor:

• Teacher Work Sample Lesson template
• Teacher Work Sample and Performance Prompts
• Teacher Work Sample Scoring Rubric
