By allowing my child to be part of the clinical experience for students enrolled in the reading diagnosis and remediation courses at Southern Utah University, I agree to the following:
1. Test results and work by students from these courses are sometimes used anonymously for other SUU courses.2. Test results and work by students from these courses may be used anonymously for purposes of research conducted by SUU researchers.
3. We may request other test results from the student's school.
4. Our test results may not be used as criteria for placement in schools.
5. Data and test results may be collected by means of observations, interviews, surveys, videotapes, photographs, audiotapes, or other appropriate means. All will be used anonymously where possible, and written permission will be sought prior to using any picture or videotapes in which my child can be identified.
I have read this document and accept it as terms for having my child be included in the clinical experience of the aforementioned students.
