An idea for a book report that correlates with math is by using the idea of Fibonacci Series. You can have them write for each number they can write that many words about the book, but for each number you would have the plot, title, author, and so on. For example:

For the first number you can have them write the author. For the second number you can have them write the title. For the third number they would write the main character. For the fourth one they could write the setting, and so on. You could have them write about the plot, their favorite part about the book, their least favorite part about the book, and so on.
More Ideas:
*Have the students demonstrate something they have learned from the book. Most books regarding math that children read are making it easier to understand the topic being read about. After the student has read the book they hopefully will understand it and in front of the class can give a two or three minute presentation on what they read about.
*Make a book jacket with an inside summary.
*Make a mobile using a coat hanger.
*Make a diorama
Have the students make a diorama around the time you are talking about measurements. Make sure they have certain measurements for the length and width of the diorama. By doing this you are reinforcing what you are teaching in math to fit with reading.
