Labs Survey Fall 2003

To assist the SUU Information Technology department in improving the services it provides in the student lab environment, your participation in this survey is greatly appreciated. In order to help show our appreciation, upon completion of the survey, your name will be entered into a drawing for your choice of a DVD player, a PDA, or free books for spring semester, each valued at around $300. Three winners will be chosen and notified on Dec. 16th. Your name for the drawing is entered separately, and in no way is linked to any written comments, or the results of your survey. Therefore, we request that you be honest and complete in your survey responses to provide us with the most helpful information as we plan for this important aspect to your educational experience.

As part of the survey, you will need to supply your student server username and password. This allows us to verify that you are a current student, that you only complete the survey one time, and to submit your name for the drawing. When the survey is submitted this information will be removed from your survey response and is only used for the drawing.
