Instructions on How to Write Practicum/Internship Papers
Please read these instructions thoroughly. Those who do not, not only produce poor papers, but miss a major learning experience.
Attendance Make-up Procedures
Since class attendance is graded it is important to make up missed classes. This handout describes procedures for doing that.
Temporary Grade Agreement
Everyone should read, sign and retain copies of these forms.
Confidentiality Form
SUU students often get to do more in practicum settings than most clinical psychology graduate students do, a very unique opportunity. The highly responsible, professional conduct of past students is what keeps the doors open to present students. Out of more than 1,000 SUU students who have taken the practicum none have committed a significant breach of confidentiality. Preserving our excellent reputation is vital for future student opportunities. Please read, sign and retain a copy of this form, and then re-read it from time to time.
Waiver of Liability Form
No significant harm has come to any practicum students in the past, but risks are naturally higher out in the “real world” than in a classroom. The University wants to be sure students are aware of the increased risk factor, and accept responsibility for the additional risks, with informed consent.

Officer/Offender Roles
This serves as a model for diagramming many other relationships.
