Directions for Completing Graduation Paperwork

Deadline for May graduation: papers to the Registrar by December 1, 2005. (For December Graduation: as soon as possible in September.)

First Phase: Get the Forms:
  1. Go to registrar’s office for the graduation paperwork packet. THEY WILL ASK YOU YOUR CATALOG YEAR. You will get:
    - the Graduation Worksheet for your catalog year
    - a Certificate of Completion
    - a pink Application for Graduation card
    - a brochure explaining the steps and fees.
    Education students, get your forms from Suzanne Brinkerhoff.
  2. Have Lea Decker in the registrar’s office run a DEGREE AUDIT. You can download one online BUT GET AN OFFICIAL ONE. Lea catches things you’ll miss.
  3. Check the Graduation information on line at www.suu.edu/graduation. The FAQs page is helpful.
Second Phase: Fill out the Graduation Worksheet
  1. Make sure you have the Graduation Worksheet appropriate for your catalog year.
  2. Fill out the front side, top part of the Graduation Worksheet.
    - If the catalog year isn’t already printed on the form, enter it.
    - Fill in your name, the date, your Academic college. [Use the abbreviation CPVA.]
    - Fill in your campus ID number (or your SSN if you don’t know your campus ID)
    - Indicate the degree you are earning (BA, BS or BIS).
    - Fill in your major. (No need to enter the emphasis.)
  3. Refer to your Degree Audit in filling out the front and back of the Worksheet.
IF YOU EARNED AN ASSOCIATES DEGREE from a Utah College or Junior College, DO NOT ITEMIZE YOUR GENERAL EDUCATION CREDITS. Go to step 8.
  1. Fill out the front side, middle part.
    - List the GE courses you have completed (Dept. prefix and course number, not title.) Fill in the number of credits and the grade. Include courses you are taking this semester and any you will take next semester. Instead of entering a grade, write the semester code (e.g., 05F for Fall 2005, 06S for Spring 2006, 06M for Summer 2006. )
    - For the General Education Core, specify the course number for MATH, ENGL and AI (American Institutions).
  2. Fill out the front side, bottom part.
    - Fill in the total SUU credits you will have on graduation (including ones you will take). DOUBLE CHECK YOUR TOTAL CREDITS IF YOU’RE CLOSE TO THE MINIMUM.
    -
    Fill in the number of transfer credits (if any).
    - Your academic advisor will review with you the number of upper divisioncredits (3000 and 4000 level courses). You must have at least 40, 20 of them must be in your major.
    - Fill in the total number of credits you will have in your major (including ones you will take). Ditto for your minor, if you have one.
  3. Fill out the back of the Graduation Worksheet.
    - List all the courses you have taken and will take for your major. Ditto for your second major if you have one.
    - List all the courses you have taken and will take for your minor if you have one.
    - IGNORE THE SUMMARY SECTION . (Most majors do not require this to be filled in. They accept the University requirement of 2.0 GPA. Business is the exception: they require a 2.5 GPA.)
    - YOU DON’T NEED TO LIST ELECTIVE CREDITS. But count them in the total number of credits.
  4. Fill out the pink Application for Graduation card.
  5. Fill out the Certificate of Completion – BOTH TOP AND BOTTOM SECTIONS. You can find the exact date of Spring graduation at www.suu.edu/academics/calendar.html
Third Phase: Obtain the Signatures

The Graduation Worksheet requires three signatures (four if you have a minor or double major). The Certificate of Completion requires only the Dean’s signature.
The pink Application for Graduation card doesn’t require any signature.

  1. Obtain the signatures in this order:
    - Make an appointment with Dan Frezza, Academic Advisor for CPVA. (If you have no questions, you can leave the paperwork and pick it up the next day.) DO NOT JUST DROP IN. Reviewing your transcript and graduation Worksheet takes about 15 minutes. Please do not expect me to do it on the fly. That leads to mistakes.
    - The Academic Advisor reviews the General Education and University requirements and signs off on them.
    - The chair of your major department reviews and signs off on your major requirements. NOTE: Theater majors, see Chris Lusk first. She initials the form, then Shauna Mendini signs it.

    Take your Degree Audit with you when you see your major department chair. This will help him/her when reviewing your major credits.

    - If you have a minor or if you have a second major, the chair of that department reviews and signs off on your minor or second major requirements.
    - Dean Byrnes signs the Graduation Worksheet last . He also signs the Certificate of Completion. (Be sure to fill out both top and bottom.) If he’s not available, you can leave them in his in-tray by his office door and pick them up later from Dan Frezza.
Fourth Phase: Deliver to Registrar
  1. KEEP A COPY OF THE GRADUATION WORKSHEET . Take the signed forms and the pink Application for Graduation card to the Registrar. Take your checkbook too and pay the fees.
