To nominate a colleague you are encouraged to complete this form and then click submit. For full consideration, all fields must be completed and submissions must be received via e-mail. A confirmation of receipt will be sent to you once the submission has been confirmed as complete and accurate.

Submissions will be reviewed twice per year – the first week of December and April.

Please explain in 250 words or less why you believe this person/group should receive the Student Services Excellence Award. Please consider the following when writing your justification:

  1. How does/did this person/group/department go above and beyond the everyday expectations associated with their role on campus?
  2. How does/did this person/group/department make a difference in the life/lives of others on our campus.

Please note that some or all of your comments may be shared with the nominee should s/he be chosen as the recipient of the award.
