Student Services Excellence Award

Submit a Nomination

To nominate a colleague you are encouraged to complete this form and then click submit.  For full consideration, all fields must be completed and submissions must be received via e-mail.  A confirmation of receipt will be sent to you once the submission has been confirmed as complete and accurate.

Submissions will be reviewed twice per year – the first week of December and April.  Deadlines for consideration are November 15th, March 15th respectively.

1. How does/did this person/group/department go above and beyond the everyday expectations associated with their role on campus?
2. How does/did this person/group/department make a difference in the life/lives of others on our campus.

Please note that some or all of your comments may be shared with the nominee should s/he be chosen as the recipient of the award.
