Diploma Replacement

Replacement diplomas are available for a $25 fee. 

If replacing a diploma that was LOST:

• Please provide your name as it appeared at graduation

If you are replacing your diploma because of a NAME CHANGE:

• Please provide a copy of your social security card or a legal document reflecting your new name

If you want to replace a SOUTHERN UTAH STATE COLLEGE diploma for a Southern Utah University diploma:

• Please provide a photocopy of your original SUSC diploma

You may mail, fax or bring your request in writing to the Registrar’s Office. Please allow 2 – 4 weeks for processing.  Please include the following information:

• Signature
• Full name and address
• Birthdate
• Daytime telephone number
• Social Security Number or Tnumber
• Year graduated

Please make personal checks or money orders payable to Southern Utah University.  If you would like to submit a credit card for payment, you must include your card number and expiration date on your request.  Visa/MasterCard/Discover/American Express cards are accepted.  Please do not send cash through the mail.
