- Security Question
- Local Address
- Phone Number
- Email Address
- Emergency Contacts
- Marital Status

- PIN: (If this is your first time logging in, use your birthdate MMDDYY)

Students who are unable to change their address online may print the following form and turn it in to the Registrar's Office, or you may email the address change to registrar@suu.edu.
