Official Transcripts
Requests for transcripts must be received in writing, as your signature is required as authorization to release your transcript. You may visit the office to order a transcript with your Photo ID, or you may fill out the Transcript Request Form below which can be faxed or mailed to us. If you are unable to access the form, you may send a letter of request with the information noted below. Please allow 2-4 business days for processing.

Any Holds on record will need to be cleared before a transcript request can be processed.

TRANSCRIPT FEES:
- Mailed First Class, $4.00 per transcript
- Faxed Transcripts, $8.00
Includes both a faxed and a mailed official copy – a faxed transcript is considered unofficial (we cannot send faxes to numbers outside the U.S.) - Priority Mail, $10.00 per transcript (2 - 3 days)
- Express Mail, $20.00 per transcript (1 day, in state / 2 days, out of state)
- Federal Express, $20.00 per transcript (overnite)
Please make personal checks or money orders payable to Southern Utah University. If you would like to submit a credit card for payment, you must include your card number and expiration date on your request. Visa/MasterCard/Discover/American Express cards are accepted. Please do not send cash through the mail.

LETTER OF REQUEST INFORMATION:
If you are unable to access the Transcript Request Form, or instead, wish to write a letter, please include the following information:
- Signature
- Full name and address
- Daytime telephone number
- Social Security Number or Tnumber [Find your Tnumber]
- Date of Birth
- Name you attended under, if different from your current name
- Address(es) where you need your transcript sent
- Approximate dates of attendance
- A note in your letter if you would like us to wait until current semester grades or degree are posted

You may view and print your unofficial transcript online (pre-1985 transcripts are not available online)

A printed unofficial transcript will not display your name.
