Documentation Guidelines

1. Diagnosis.
2. Testing and interpretation.
3. Limitations in major life activities.
4. Recommendations while the student is attending college and why these recommendations are given.
5. Documentation must be recent:
   1. Within 3 Years for Learning Disabilities
   2. Within 3 years for Physical Disorders
   3. Within 6 Months for Mental Disorders
6. Testing must be performed by a professional psychologist or other specialist.
