Grant Report Form Instructions

Purpose

The UGRASP Grant Report Form helps to establish that UGRASP funds were used for their intended purpose.  The information obtained on this form, along with the UGRASP Grant Application Form, will also be used to determine the relative success of funding different types of requests. 

Who needs to complete a UGRASP Grant Report Form

In the case of individual student or group research projects, the UGRASP Grant Report Form: Student Version is completed jointly by the mentor and his/her student(s).  Submission of the completed form is the responsibility of the mentor.

In the case of a class research project, the UGRASP Grant Report Form: Instructor Version is completed and submitted by the class instructor.

When to submit a UGRASP Grant Report Form

The UGRASP Grant Report Form must be submitted within one week of the last day of classes for the semester the funds were distributed, regardless of whether the project is complete or not.

A project is considered complete once its methods/procedures have been carried out (as outlined in the original application form) and the results of the project have been disseminated.  The dissemination requirement may be satisfied through venues ranging from a formal class presentation to a peer reviewed professional publication.

A project is also considered complete if circumstances require that it be abandoned, for whatever reason.

In the event the project is not complete upon submission of the UGRASP Grant Report Form, a second UGRASP Grant Report Form will need to be submitted within one week of the “anticipated project completion date” indicated on the initial UGRASP Grant Report Form.

Who to submit the form to

The form should be completed online, printed, and signed by all required parties.  The form should then be sent via campus mail or faxed to:
