Clinical Documentation

ALL clinical documentation for Spring Semester 2018 (this includes any documentation expiring January 8, 2018 - April 26, 2018) must be updated and turned into the office manager by noon on Friday, December 15, 2017. Paperwork can be hand delivered or mailed to SUU Department of Nursing, Attention: Teresa Higbee, 351 West University Blvd., Cedar City, UT 84720 or e-mailed to higbeet@suu.edu

Application Process

  1. RN Candidates, Request an SUU Transcript from MySUU Portal, select "After Degree is Posted" and send to doplbureau7@utah.gov.
    PN Candidates, pick up a Letter of Equivalency from the Department of Nursing after passing the PN-Predictor (to be enclosed with your application to DOPL).
  2. Register and pay Pearson Vue.
  3. Once transcripts are sent (or PN-Letter of Equivalency is obtained), send to Occupational and Professional Licensing (DOPL) the completed application, required fee and completed fingerprint cards.

(Please note, test must be taken within 30 days of DOPL receiving application!

NCLEX Examination Registration

LPN by equivalency test school code US38900000 (application must be in to the state board within 30 days of the date of the letter of equivalency)

RN school code US38500000

Please note: PN assessment exam for SUU nursing students (must complete level 2 successfully) will only be offered twice a year; once in May and December. Next exam will be on Monday, December 18, 2017, 9:00 a.m.-12:30 p.m., SCA 011. Fee for exam is $55 for proctored assessment or $85 for proctored assessment and Practice Exams. Students must pay and pre-register through the nursing office no later than Noon on November 10, 2017 (fees are non-refundable and are subject to change). Contact the Nursing Office for additional information. Students can pay LPN fee online (Bring receipt to office after paying).

Clinical Documentation: Spring 2018 Clinical Documentation is due to Teresa (SCA 108C) by 12:00 p.m., Friday, December 15, 2017 

Intermountain Healthcare ID badge pictures:  Students will go to Cedar City Hospital, Human Resources Department (HR), located in the Basement (follow the signs), at 1303 N. Main Street, Cedar City, UT, Monday through Friday, between 0830 - 1630.  Let HR know that your are an SUU Level 1 nursing student and need to get a picture taken for a student clinical ID badge.  NO HATS!  Pictures must be taken by December 15, 2017.

Register for the following NURS courses: 3120, 3130, 3140, 3160 and one of the 3135 sections.
You must be flexible – there will be class or clinical experiences everyday; Monday through Friday, in this first semester.

Classes are Mondays and Tuesdays (check the posted schedule); Clinicals/Labs are on Wednesdays, Thursdays & Fridays (varying times depending on group)
