Canvas Requests

The forms listed below may be used to request various changes to Canvas courses.

Only SUU Faculty and Staff members will be able to access the forms.


This request is used to provide access to both academic and non-academic (i.e. staff trainings, committees, or meeting groups) Canvas shells, for TAs, co-instructors, meeting participants, training enrollees, observers, and course designers. 

Note: TAs and Graders are required to take FERPA Certification Training every semester before they will be added to the course.


This request is for faculty who wish to combine multiple sections of their courses into one course in Canvas (ex. Faculty teaching 2 sections of ENGL 1010).


This request is for faculty or staff who have need of a Canvas shell that is not part of the SUU course catalog. For example, shells can be used for department meetings, committees, training, and more.


This request is for faculty and/or course designers who have completed designing an online course. This request will initiate the review of your course by a member of the SUU Online Team.
