Annual DCHP Review Checklist

Following the DCHP template ERM will review the following as part of the Annual DCHP Review:

• Review list of departmental PIs, Laboratory Supervisors, Laboratory Workers (employees).
• Review of training compliance for all PIs, Laboratory Supervisors, and Laboratory Workers (employees).
• Review Lab specific student safety training.
• Record keeping.
• Lab specific training compliance audit for all PIs, Laboratory Supervisors, and Laboratory Workers (employees).
• SOP review.
  • Hazard identification and mitigation controls.
• Lab and equipment inspection.
• PPE evaluation.
• SDS and chemical inventory.
• Review of past incidents (what changes/improvements have been made as a result).
• Waste management.
• Chemical storage.
