The following is a list of forms that you may need to fill out at some point during your studies at SUU.

Statement of Understanding

SUU students who will be receiving VA Benefits must sign this form to verify understanding of their responsibilities.

Residency Application

Please fill this out and submit if you are considered a non-resident student and qualify for in-state residency.

CHAMPVA Enrollment Certification

Children between 18 and 23 years of age must submit a school enrollment certification letter confirming full-time student status. This is required in order to continue eligibility for CHAMPVA benefits.
