Disability Documentation Guidelines

The DRC at Southern Utah University follows ADA (American with Disability Act) mandates. To provide any accommodations, we need formal documentation done by a professional stating the following:

1. Diagnosis using the respective criteria depending upon the type of disability
2. Substantial Limitation in major life activities, limiting the individual's learning and access to the classroom. These limitations must be personal and specific.

You may hand deliver documentation to our office at SUU in the Sharwan Smith Student Center room 206 or send it through our secure file upload system to one of our staff members.
