23-24 Health Insurance Changes Memo
Fill out the Change of Benefits form using the qualifying event 'Open Enrollment' to make any changes to your benefits for the 2023–24 fiscal year.
UMR (United Health Care)
Medical benefits are with UMR. Pharmacy benefits are with MotivRx. The information for both coverages is on one card that comes from UMR.

MotivRx is committed to helping you lower your prescription costs. This means if you or any covered dependent is taking a high-cost medication, including insulin, we may be able to lower or eliminate your monthly out-of-pocket costs.
Work with MotivRx to lower prescription costs
- Formulary has some things that are always denied (not just by MotivRx) and some things that have a “soft” denial while you work with MotivRx.
- MotivRx will help you get the medication you need at a cheaper cost for you and the plan.

Child Care Subsidy
Eligibility requirements for this program are that you must have a dependent child as defined by the IRS, and you will submit receipts from a licensed childcare provider as determined by the IRS for reimbursement.
Employees must fill out the SUU Dependent Care Expenses Form and send it to the benefits assistant, Gabby Cox, through her secure link.
Employees wishing to receive the subsidy every year will have to fill out the request form each fiscal year. The funds will be directed to National Benefits Services (NBS) for administration as a regular dependent care FSA.

The Standard Life Insurance
At open enrollment, employees who are enrolled in basic and supplemental life insurance may increase their supplemental coverage for themselves and their spouses.
Increases can be $10,000 or $20,000 until the employee reaches guaranteed coverage of $250,000 and the spouse reaches guaranteed coverage of $50,000.
If you would like coverage amounts greater than guaranteed coverage, you may apply for coverage up to $500,000 for the employee and $100,000 for the spouse by filling out the online medical information and application.
