Level Two Nursing Student

Hurst Review (Three-Day Live NCLEX-RN Review):  Tuesday, 04/18/2023 - Thursday, 04/20/2023, 0900 - 1730; Location: Hunter Alumni Center, Shooting Star Room.  All level 4 students will be registered by Nursing Staff.  Registration and attendance is mandatory, expectations will be explained the first part of the semester.

Register for the following NURS courses: 4430, 4440, 4550 and one of the 4435 and 4555 clinical course sections.

Program Codes needed to register for exam (NCSBN)
  • RN school code: US38500000
  • LPN by equivalency test school code (PARTIAL RN EDUCATION TAKING PN): US38909700 (L2 students that want a LPN license:  application must be in to the state board within 30 days of the date of the letter of equivalency)

NCLEX-RN Application Process:

  1. Take & Pass the NCLEX (Create an NCLEX account through Pearson Vue and pay the $200.00 testing fee).  NCLEX Examination Registration (NCSBN).  Program Codes (for registration).

  2. Licensure (After successfully passing NCLEX-RN).  Request an SUU Transcript from   MySUU Portal, select "After Degree is Posted" and send (via 3rd party clearinghouse by the SUU Registrar's Office) to doplbureau7@utah.gov

  3. Once transcripts are sent, send to Department of Occupational and Professional Licensing (DOPL) the completed application, required fee and completed fingerprint cards. Fingerprint information (DOPL).

Please note, test must be taken within 30 days of DOPL receiving application!

NCLEX-PN Application Process:

  1. Take & Pass the NCLEX (Create an NCLEX account through Pearson Vue and pay the $200.00 testing fee).  NCLEX Examination Registration (NCSBN).  Program Codes (for registration).
  2. Licensure (After successfully passing Level two and ATI PN-Predictor). Pick up a Letter of Equivalency from the Department of Nursing after passing the PN-Predictor (to be enclosed with your application to DOPL in lieu of transcripts) on or after date given at Predictor.
  3. Send PN-Letter of Equivalency is obtained to Department of Occupational and Professional Licensing (DOPL) the completed application, required fee and completed fingerprint cards.  Fingerprint information (DOPL).

Please note, test must be taken within 30 days of DOPL receiving application!

Please note: PN assessment exam for SUU nursing students (must complete level 2 successfully) will only be offered twice a year; once in April and December. Next exam will be on Friday, April 21, 2023, 9:00 am - 12:30 pm, SCA 011. Fee for exam is $65 for proctored assessment or $115 for proctored assessment and Practice Exams. Students must pay and pre-register through the nursing office no later than Noon on February 17, 2023 (fees are non-refundable and are subject to change). Contact the Nursing Office for additional information.

Students can pay LPN ATI fee online (Bring receipt to office after paying).
