ISS Request to Add Dependent

This form is to be completed by students who are requesting that dependents (Spouse, Daughter, and/or Son) be added to their immigration record.

Please note: adding dependents requires proof of funds to cover $8,000 for a spouse and $4,000 per child single and under 21 years old.
